Dupilumab (Dupixent)

Also known as: Dupixent

Dupilumab is a targeted biologic injection used for certain conditions affecting the skin, airways and sinuses. It works on a different inflammatory pathway from many arthritis medicines. Let me explain what it does and which symptoms deserve a review.

Calm, careful medication review by a consultant rheumatologist.

Let me explain

What this medicine is

Dupilumab is a monoclonal antibody: a specially designed protein that attaches to the IL-4 receptor alpha. This blocks signals from IL-4 and IL-13, which contribute to type 2 inflammation. It is given by an injection under the skin.

Why this medicine may be discussed

Uses and context

The Malaysian prescribing information reviewed for this guide lists moderate to severe atopic dermatitis (eczema) from 6 months of age when prescription topical treatment is inadequate or unsuitable; add-on maintenance treatment for severe type 2 asthma from age 12, and maintenance treatment for oral corticosteroid-dependent asthma; add-on treatment with nasal corticosteroids for adults with severe chronic rhinosinusitis with nasal polyps inadequately controlled by systemic corticosteroids and/or surgery; and moderate to severe prurigo nodularis in adults when prescription topical treatment is inadequate or unsuitable. Suitability depends on the diagnosis and specialist assessment.

Rheumatology perspective

How this relates to rheumatology care

Dupilumab is not an established treatment for rheumatoid arthritis, psoriatic arthritis, lupus or EGPA. Its relevance to rheumatology includes assessing new joint symptoms or symptoms suggesting another inflammatory condition during treatment. New or worsening joint pain, stiffness or swelling should be reported. Psoriatic arthritis has also been reported after treatment. Persistent symptoms may need rheumatology assessment, and I am happy to review them and explain the options.

Safety checks

What safety checks may matter

Review previous allergic reactions, eye problems and current medicines.

Check for parasitic worm infection and treat an existing infection before starting.

Review vaccinations; avoid live vaccines during treatment.

Discuss pregnancy, pregnancy planning and breastfeeding.

Agree how treatment response and new symptoms will be reviewed.

Routine laboratory monitoring is not universally required for dupilumab itself, though tests may still be appropriate for your underlying condition, symptoms or other medicines.

When I would like you to seek help

Side effects and red flags

If you have emergency symptoms such as severe allergic reaction, breathing difficulty, chest pain, stroke-like symptoms, severe abdominal pain, vomiting blood, black stools, sudden vision loss, severe infection symptoms, or severe weakness or confusion, please seek urgent medical care first rather than waiting for a WhatsApp reply.

Facial or throat swelling, severe breathing difficulty or collapse: seek emergency care immediately, do not wait for a WhatsApp reply.

Eye pain or changes in vision need urgent assessment; report any new or worsening eye symptoms promptly.

Worsening breathing, an unusual rash, chest pain or new numbness or weakness, especially during steroid reduction. Serious eosinophilic conditions, including eosinophilic pneumonia and EGPA-like vasculitis, have been reported, particularly in asthma patients, although a causal relationship is not established.

Persistent or worsening joint pain, stiffness or swelling.

Practical cautions

Things worth knowing day to day

Possible side effects include injection-site reactions, eye inflammation or dryness, and cold sores.

Home injection may be possible after training.

Follow the prescribed schedule; the regimen depends on the condition, age and sometimes weight.

Continue prescribed asthma treatment. Dupilumab does not relieve a sudden asthma attack.

Do not abruptly stop or reduce steroid medicines.

Keep syringes refrigerated at 2 to 8°C in their original carton. Do not freeze or shake.

The Malaysian syringe leaflet permits up to 14 days at no more than 25°C. Malaysia's outdoor temperatures may exceed this, so follow the supplied leaflet and obtain travel-storage advice.

Ask the prescribing team about missed doses.

Frequently asked

Questions patients often ask me about this medicine

No. It is a biologic medicine targeting specific inflammatory signals. Any reduction in existing steroid treatment needs a supervised plan.
