Health Assessment Questionnaire (HAQ)
The Health Assessment Questionnaire (HAQ) measures how easily you can perform common daily activities. Answer all 20 questions to receive a score from 0 to 3. Lower scores indicate less difficulty with daily activities.
How the scores work
- 0 = No difficulty
- 1 = Some difficulty
- 2 = Much difficulty or requires assistance/device
- 3 = Extreme difficulty or unable to do
