2012 ACR/EULAR Classification Criteria for Polymyalgia Rheumatica

Apply in patients ≥50 years with bilateral shoulder aching and abnormal ESR/CRP. Careful exclusion of mimics required.

Entry criteria (all required)

Age ≥50 years

Bilateral shoulder aching

Abnormal ESR and/or CRP

Clinical scoring

Morning stiffness >45 minutes+2

Hip pain or limited range of motion+1

RF and/or ACPA absent+2

No other joint involvement+1

Optional ultrasound criteria

At least one shoulder with subdeltoid bursitis, biceps tenosynovitis or glenohumeral synovitis AND at least one hip with synovitis or trochanteric bursitis+1

Both shoulders with subdeltoid bursitis, biceps tenosynovitis, or glenohumeral synovitis+1

Entry criteria not met

PMR classification requires exclusion of mimics including elderly-onset RA, infection, malignancy, and other inflammatory conditions.
